The PREEMPT protocol is a carefully designed treatment plan that targets the muscles and nerves involved in migraine pain. Botox® is injected into specific muscles of the head, neck, and shoulders to reduce the release of pain-signaling chemicals and decrease the sensitivity of the nerves that contribute to migraines.
Unlike medications that treat a migraine after it begins, Botox® is a preventive treatment, helping reduce both the frequency and severity of migraine attacks over time.
The standard PREEMPT protocol includes 31 small injections across 7 muscle groups, with a total of 155 units of Botox®. Depending on where your migraines are most severe, additional “follow-the-pain” injections may be recommended, bringing the total dose to up to 195 units.

During your consultation, we will review your migraine history, previous treatments, and overall health to determine whether Botox® is right for you.
Your appointment typically takes 15–20 minutes. Using a very fine needle, Botox® is placed into the targeted muscle groups. Most patients describe the injections as quick, with only mild discomfort.
After treatment:
Many patients begin to notice improvement within 2 to 4 weeks, although the greatest benefit often develops after the second treatment cycle. Botox® treatments are generally repeated every 12 weeks to maintain consistent migraine prevention.
